Use this form whenever your student must take medication while at school. Please note that all medicine must be brought to school by an adult- may NOT be transported on the bus (exceptions are albuterol inhalers, epipens, and insulin. There are special forms required for these meds so please contact your school nurse for assistance in completing the necessary paperwork). With the above exception, all medication is kept in the school clinic office. Medications prescribed by your healthcare provider to be taken three or fewer times per day should be taken at home. Call the school nurse with any questions about medications your student may need.
This form is to be completed in the event your student has an allergy to any food. The form is completed jointly by you and your child's healthcare provider. It must be updated annually.
If your child has been diagnosed with asthma this form must be completed by you and your child's healthcare provider annually. Because the School Nurse is shared between several district schools, it is imperative that we have this completed, up-to-date form on file for your child so that in the event of an asthma 'attack' school personnel will know exactly how to proceed until the nurse arrives. Please help us help your child by keeping us updated on his/her plan of care.
This is the form required in order for your student to participate in any extracurricular sports. It must be received by the school prior to the first day of tryouts in order for your student to participate. Any student who has not provided a sports physical completed by their healthcare provider will be excluded from play, for example, begins before the start of school in the fall.
Page one contains parts A. Part A must be completed (updated) at least every 12 months. Parent/guardian and student complete & sign parts A. Page two , Part B requires a physical exam by a licensed healthcare provider (physician, nurse practitioner, physician's assistant). Part B is required at least every 24 months.
Our school district is very fortunate to
have in place a weekly fluoride swish program. Fluoride has been proven to be
highly effective in the prevention of tooth decay. Once a week participating
children swish a pre-measured, individually packaged, 1/3 ounce fluoride
solution for 60 seconds, spit it back into their individual container, wipe
their mouths with a napkin which they then tuck into the container, which is
then disposed of in a separate plastic bag. The fluoride comes in a variety of
flavors, including unflavored. I encourage everyone to take advantage of this
